1) Any referral information and x-rays. If another health professional referred you to our office, please bring any referral paperwork they provided along with applicable x-rays.
2) A list of current medications. Please provide a list of all medications you are currently taking.
3) Insurance paperwork. If you have medical or dental insurance, please fill out the appropriate paperwork.
